✅ What to Include in Your Ticket

Please copy and complete the fields that apply to your issue:

  • Cycle’s name:
  • Status: (e.g., draft, pending launch, running, stopped, ended)
  • Review’s type: (e.g., self, manager, peer, upward, custom)
  • Rules applied: (e.g., “share review when all reviews are submitted”)
  • Owner: (person responsible for the cycle)
  • If it's a issue related to goals add the goal type
  • URL link to the cycle/Survey
  • Name/Email of people involved in the issue, .e.g. if its a manager review cycle, the manager's and report's name and email
